ANALYSIS OF ACCIDENT INVESTIGATION PRACTICES IN MERCHANT SHIPPING COMPANIES
 
One of the focal points in maritime sector is to increase safety by decreasing ship accidents and incidents. The nature of onboard operations increases the likelihood of hazardous occurrences, which might cause significant damage to the economy, human life, and the environment. According to the IMO requirements, to avoid such occurences or to minimize their impacts, marine accidents/incidents are reported and analyzed immediately. Through investigating ship accidents, it is aimed to specify the underlying root causes that lead to hazardous situation and to implement corrective and preventive actions. The occurence analysis is carried out according to the procedures of shipping companies. However, the structure of the forms used for analysis by shipping companies is quite different from each other. In this research, the structure of analysis forms of different international shipping companies were reviewed. It was revealed that while some shipping companies use a form as short as one page, some shipping companies use forms designed in detail for in depth analysis of complex accidents. Within the scope of this research, international regulations and standards on hazardous occurence analysis were examined, and shipping company procedures were reviewed. The significance of the structure of accident analysis forms in maritime safety was discussed. In conclusion, the need of a standardization in the structure of form was emphasized.
